appointment can take place at our office in
Virginia Beach, Chesapeake or Norfolk Virginia,
or "VIRTUALLY" anywhere. We now offer
e-consultations
and/or tele-consultations in
the comfort of your home or workplace.
- After
scheduling your appointment, you will receive
an email confirmation and nutrition consultation
forms from us.
- As
an added bonus, you will receive a secure login
to access our client's only section (free for
30 days).
- If
you are scheduling an appointment from a physician's
referral, and plan to file for reimbursement
from your insurance company, please have your
physician fax your medical diagnosis and current
pertinent lab work along with our physician
referral form to (757) 499-2017. All
information is kept confidential

Policies
and Procedures:
Appointments
Individual appointments are reserved just
for you, at a specific time. You may reschedule,
change or cancel your nutrition appointment day
and/or time by calling (757) 499-2018 up to 48
hours before your scheduled appointment time. If
you are late for your appointment, the appointment
will end at the originally scheduled ending time.
If you miss an appointment you forfeit time missed,
if you are a no call/no show, in order to schedule
a new appointment, a service fee will be assessed
prior to rescheduling. However, your allocated
time in the package will be honored.
Fees and Payment
Full payment for a nutrition
service package is expected at or prior
to the first appointment. We accept
cash, checks and most major credit
cards. No refunds are provided. In
many instances your insurance plan
will reimburse for MNT. MNT is a new
benefit for Medicare
Part B recipients,
diagnosed with type 1 or 2 diabetes,
gestational diabetes, non dialysis
kidney disease, or post kidney transplant..

Consent for Services
All clients are asked to sign
a consent
for services prior to your
first appointment, agreeing that you
understand and will adhere to our policies
and procedures. Your signature also
indicates that you accept nutrition
counseling services and have been offered
a copy of our privacy notice. We look
forward to helping you achieve your
health and nutrition goals.
